Meet Jacob Hardt
A steady hand for families in their most vulnerable hours.
People often reach out when conversations have broken down and nobody knows what to do next. I start by listening, understanding what's happening, and building enough trust to move forward together. My work combines ten years of intervention experience, coaching, and lived experience. I stay involved as we turn difficult conversations into practical next steps.

What to expect
How it works — step by step.
An intervention
- 1
The first call
We talk privately about what's happening, what's been tried, and what support is needed. From there, we identify everyone close to your loved one — family and friends.
- 2
One-on-one conversations
I sit down with each person separately. Everyone gets to speak freely, and I get to know the whole picture from every side.
- 3
Coming together
Just before the intervention, we all meet as one group — getting on the same page and practicing the language we'll use, with love, compassion, and strength.
- 4
The intervention
An invitational intervention, shaped in the moment to your situation. Flexibility is crucial — no two go the same way — and I guide it as it unfolds.
- 5
The yes
When we get a yes, we stop there and move straight toward treatment — protecting the moment, not pushing past it.
- 6
The weeks after
I stay with your family as the change takes hold — the return home is often when support matters most.
Life coaching
- 1
Discovery call
We talk about where you are, where you want to be, and whether we're a good fit.
- 2
A clear plan
Together we set goals and a rhythm of regular sessions — in person, by phone, or video.
- 3
Support between sessions
Accountability and check-ins so progress keeps moving between our conversations.
No two situations are alike — every plan is shaped around the person and the family in front of me.
How the work fits together
One person to call — a whole circle of support.
You work with me directly. When you need more people around you — a therapist, a psychiatrist, a high-quality assessment program, a concierge Break Free journey — I bring the right qualified people together and keep everyone moving in the same direction.
Strategist
Who this is for
Wherever you are, there is a next step.
I don't just focus on the substance or the behavior. I focus on the human being behind it — and the family standing around them.
People becoming their best self
You don't need a crisis to work on yourself. I also coach people who are already doing well and want to become exceptional.
